Billing for urgent care
Urgent care is volume. A clinic can see more patients in a weekend than a primary care office sees in a week. Billing has to keep the same pace, or the accounts receivable is already old on Monday.
What makes urgent care claims different
Visits are new to you even when the patient has a primary physician. Procedures — sutures, splints, incision and drainage, rapid tests — happen in the same encounter as the visit. Payers want the procedure and the visit coded so they are not paying twice for the same work, and they want the diagnosis that supports each line.
Place of service and after-hours rules vary by contract. We bill what the contract and the note support. A case-rate contract is not billed as a stack of fee-for-service lines.
Where the money leaks
Eligibility, because patients arrive without an appointment and the card is expired. Authorizations for studies you send out the same day. Denials for a missing modifier on a procedure done with the visit. Patient balances that nobody statements because the front desk is triaging.
We check eligibility on arrival, code the visit and the procedure from the note, and statement the patient share after the payer responds. Days in accounts receivable are watched weekly, not at the end of the quarter.
