Medical coding for office visits
Coding is how a visit becomes a billable claim. MedFlow codes from the provider’s note, then a person confirms the codes before the claim is built.
What the coder works from
The note, the patient’s age and coverage, and the fee schedule for that clinic. Suggestions use the CMS ICD-10-CM code list and NCCI edits that MedFlow loads into the product. The software shows the sentence in the note that supports each code. A coder marks each suggestion correct or wrong and can change it.
We do not code from a superbill alone when the note says something different. If the note is missing a side, a time, or a medical-necessity sentence, we send a question back to the provider instead of guessing.
Office specialties we code
Evaluation and management for primary care and internal medicine, including Medicare wellness visits. Urgent care visits with procedures done in the same encounter. Behavioral health diagnostic interviews and psychotherapy, including when the visit was by video and the note says so.
Inpatient hospital coding and surgical coding for the operating room are outside this service.
Why small practices feel this first
An under-coded established visit is quiet: the claim pays, just at the wrong level. An unsupported code is loud: the payer denies it or asks for the note. We would rather ask the provider one question than file a claim we cannot defend.
Included
- ICD-10-CM diagnoses linked to each procedure
- CPT and HCPCS, including modifiers the payer requires
- NCCI and medically unlikely edit checks before submission
- A question to the provider when the note is not enough
